The Unique Disability ID issued for the Persons with Disability by the Department of Empowerment of Persons with Disability, Ministry of Social Justice and Empowerment, Government of India,

The project is being implemented by the Ministry of Social Justice and Empowerment, Government of India, with a view of creating a National Database of the Persons with Disabilities and to procide a Unique Disability Identity Card to the person with the disability

As the Unique Disability ID Card will help to encourage transparency, efficiency and ease of delivering the Government benefits of various Schemes to the person with disabilities, and also to ensure the uniformity among all

WHY UDID?

The online availability of information of the Person with Disabilities across country will be made available through a web application, The online filing and submission of registration application form for disability certificate/ Universal ID card will be easily accessible to all

The offline applications may also be accepted and subsequently digitized by the concerned agencies,

It will help Quick Assessment process for calculating the percentage of disability, It avoids duplication of Personality of Disability data, Easy renewal and update of information by Person with Disabilities

BENEFITS OF UDID

The UDID Card minimize the risk of Persons with disabilities making the multiple copies of documents, maintain those, and carry multiple documents whenever required, as the card will contain all the necessary details,

The UDID card will be the single document of identification, verification of the disabled for availing the Schemes and other benefits in future. The UDID Card will also help in stream lining and also the tracking of the physical and financial progress of beneficiary at all the different levels 

APPLY ONLINE GUIDE

Step 1 : Person with Disability Click on register in UDID Web Portal (CLICK HERE)

Step 2 : Using credentials get log in to system and click “Apply online for Disability Certificate, Before fill the applications carefully read all the instructions and fill the online application.

Step 3 : Upload recent colour passport photo and other requisite documents like Income certificate, Identity Proof and caste certificate  as required.

Step 4 : Submit the data to CMO Office, Medical Authority.

Step 5 : CMO Office, Medical Authority will verify the date

Step 6 : CMO Office, Medical Authority will assign the concerned specialist for assessment of Person with Disability

Step 7 : Specialist Doctor assesses the disability and gives the opinion on type, Percentage of disability.

Step 8 : Finally the Medical Board reviews the case and assign the disability percentage. CMO Office will prepare the Disability Certificate and generates UDID 

Step 9 : UDID datasheet goes for UDID Card printing and Card is dispatched
