The Karnataka State Government has released the Gazette notification regarding the revised instructions regarding the Policy Frame Work for implementation of the new Karnataka Arogya Sanjeevani Scheme-KASS

Karnataka State Government has initiated to implement Karnataka Arogya Sanjeevani Scheme for the State Government Employees in collaboration with the Suvarna Arogya Suraksha Trust (SAST), As per the notification released the family dependents to be considered Husband, Wife, Parents (Father and Mother) of Employee and Children including adopted 

Form of Application to be submitted by eligible Government Employee for enrolment into the Karnataka Arogya Sanjeevini Scheme (KASS) has been released, the application form to be submitted through Head of Office (Office where Service Register of the Employee is maintained / Reporting Officer to the DDO

The DDO are directed to enter the details of employees in HRMS, the documents to be submitted along the application form by the employees are Photo of all dependents (State Government Employee to sign and enter date on every photo), Aadhar Card, Date of Birth Certificate, Pay Slip and other), Along with the application form the employee to submit the self declaration
